Clinical Pharmacists in Practice

Hi my name is Haseeb Khan and I am the new Practice Pharmacist that has joined the team here at Altrincham Medical Practice. In 2015 the NHS has deployed a huge drive to bring Clinical Pharmacists into General Practice to reduce the burden and pressures on GPs. I will be able to support this by bringing expert knowledge about medicines and disease states and can draw on experience from working in hospitals and community pharmacies across Manchester.

I will be working here from Wednesday afternoon to Friday and will be running patient facing clinics at the practice, answering questions about medicines, carrying out medication reviews and discussing how I can improve your treatment and make sure it is right for you. I am particularly suited to supporting the transition of care of older people who have recently been discharged from hospital.
